CareCHOICES Hospice and Palliative Services, LLC is seeking a very dynamic and hospice experienced registered nurse Director of Patient Care Services for the clinical operations leadership of our hospice organization.
To be considered for this important position with our experienced leadership team, the ideal candidate must have the following:
A strong desire to provide outstanding hospice and specialty program patient care management with the ability and leadership skills to inspire an excellent team to do the same!
4-5 years of progressive hospice leadership responsibility for directing regulatory compliance with Medicare and Department of Public Health for a hospice organization.
Extensive experience with and compliance to the hospice Medicare Conditions of Participation required.
Strong attention to detail, organization and the ability to collect, audit, review and analyze hospice data and indicators, monitor and track trends for IDG education.
Exceptional computer skills with special advanced/expert level emphasis on MS Office and EXCEL for statistical data collection to ensure outstanding report generation capacity.
Demonstrated success in working independently, utilizing high level critical thinking and analysis skills.
Outstanding interpersonal, verbal and written communication skills, as well as group presentation experience.
Exceptional ability to inspire excellence in all levels of hospice IDG staff through ongoing teaching, orientation and team building.
Extensive experience with EMR (electronic medical record) since CareCHOICES utilizes MatrixCare computerized clinical point of care EMR.
Experience with ADR and regulatory audit process.
