POSITION SUMMARY
The RN Outpatient Case Manager will serve as a bridge between the healthcare professional in the hospital setting and the patient and their caregiver. In the role, the Nurse Outpatient Case Manager will provide information and guidance to the patient and caregiver for an effective care transition, improved self-management skills and improved patient-physician communication. The Outpatient Case Manager will function as a guide for patients and caregivers, addressing critical issues and self-management tasks rather than providing direct care. The goal of the Outpatient Case Management Registered Nurse RN is to improve care transition by providing patients and caregivers with education, identification of referrals to other needed healthcare resources and support that promotes knowledge and self-management as they transition from hospital to home and other post-acute care.
