    The role you’ll contribute:

    The Medical Records Data Entry Specialist is responsible for the preparation of the medical record for completion of the annual wellness visit for assigned patients and/or the completion of chart abstraction. The annual wellness visit preparation will require the contact of assigned patients to schedule, interview and document the Medicare Annual Wellness Visit. The chart abstraction will require the location and retrieval of information from the appropriate medical record and prepare the data for presentation to the requester.

    • Contacts assigned patients via telephone and greets patient appropriately, accurately updates, verifies, and enters patient demographics (including insurance and billing) and processes/scans all forms into the appropriate systems
    • Screens assigned patients utilizing standardized templates, e.g. date of last cancer screening, mental health screening, etc.
    • Prepares electronic medical chart for patients already on the schedule in need of an Annual Wellness Visit add-on
    • Provides education as necessary to patient regarding patient portal use, Annual Wellness Visit, Preventative Services and follow-up (non-clinical) actions as needed
    • Documents appropriately within the office's designated system to ensure regulatory requirements are met including, but not limited to, MIPS, PCMH, and other quality programs
    • Chart Abstraction:
    • Reviews patient electronic medical record in one system and confirms and updates data in subsequent system to ensure accuracy and completeness of medical record
    • Abstracts health information from patients’ paper and or electronic medical charts, as needed
    • This will be a hybrid role which will allow you an opportunity to transiition into a non-clinical support role.

    Qualifications


    The expertise and experiences you’ll need to succeed:

    • High School graduate or equivalent
    • Diagnosis coding and medical billing experience preferred
