Medical Records-Tech I - Full Time
POSITION SUMMARY:
Monitors and responds to requests for release of patient health information ensuring compliance with all federal, state and healthcare regulatory standards, including HIPAA. Coordinates medical records requests with Risk Management, Privacy Officer and/or Revenue Cycle. Assembles and analyzes medical records for completion.
POSITION REQUIREMENTS:
Minimum Education:
- High School Diploma or equivalent.
Minimum Work Experience Required:
- Good organizational and multitasking skills.
- Professional customer service skills.
- Knowledge of computer applications, Intermediate skills in Excel and Word required.
Minimum Work Experience Preferred:
- Minimum of 2 years office or healthcare experience preferred.
- Knowledge of patient privacy regulations, certification preferred.
- Ability to read and communicate in English Bilingual preferred.
POSITION ESSENTIAL FUNCTIONS:
Medical Information
- Maintains and responds to requests for release of patient health information ensuring compliance with all regulatory standards to include HIPAA.
- Effectively communicates special circumstances to Risk Management and/or Privacy Officer.
- Processes request ensuring accuracy and completeness of information, correct patient, appropriate encounters/visits/admissions etc.
- Ensures proper authorizations are obtained.
- Keeps accurate correspondence.
- Generates invoices as needed.
Tracking
- Assists in maintaining a tracking system to ensure that responses to audit requests occur within established time frames without exception.
- Assists the Revenue Cycle Leadership in effectively coordinating audit requests associated with any government and/or private payor audit request including but not limited to Recovery Audit Contractors (RAC), Medicaid Integrity Contractors (MIC), Zone Program Integrity Contractors (ZPIC), Comprehensive Error Rate Testing (CER1), Payment Error Rate Measurement (PERM), Medicaid Integrity Program (MIP) and/or Medicare Risk Adjustment (MRA).
Records
- Assembles and analyzes medical records ensuring assessment and identification of all required elements mandated by regulatory agencies and CVMC policies.
- Ensures all loose filing is assembled in the appropriate record.
- Ensures all charts are complete and ready for coder.
- Reviews medical records to ensure chart is complete prior to placing permanent files.
- Collect and deliver charts and documents to and from the HIM Department to various facility locations.
- Identifies all medical record deficiencies, assigns deficiency to the appropriate physician and tags all items for review by the physician.
HIM Liaison
- Acts as the HIM customer service liaison.
- Answers incoming phones in a courteous and professional manner.
- Greets: customers in a manner conducive to the CVMC Code of Conduct.
- Faxes requested documents as needed while maintaining all standards of privacy.
Scheduling
- Effectively coordinates scheduled daily activities to ensure productivity standards are met and work is completed in a timely manner.
- Oversees the filing/scanning of patient health documentation in the patient's record into the appropriate location, ensuring accuracy and completeness.
- All available documentation is filed in a timely basis.
