Hospice of Central NY is in need of a Health Information/Medical Records Coordinator in our Liverpool
office.
The Health Information Coordinator must:
Possess a strong working knowledge of ICD-10 Medical Coding
Have the ability to communicate and interpret patient records standards.
Have the ability to work with and function as a team member of an interdisciplinary team at all levels
within the Organization.
Have the Ability to coordinate ongoing activities and special projects
Be proficient in the use of techniques, tools and software, with strong verbal communication skills
and the ability to interact positively in a variety of settings, with a diverse staff, volunteers and with
all members within the community we serve.
Possess a valid NY State Driver’s License and reliable transportation.
Be proficient in managing record releases and subpoenas
The Health Information/Medical Records Coordinator exercises mainly independent judgement under general
spuaprteircvipisaitoens ;i n setting work objectives and coordinates work and resources within established guidelines.
The main job duties of the Health Information/Medical Records Coordinator consist mainly of:
Managing patient records information per agency standards, including organizing filing and archive
system to facilitate the expeditious retrieval of information; entering patient data timely and
accurately, initiating patient record on admission.
Processing incoming mail, chart documents, nursing and psychosocial assessments, and on-call
paperwork.
Monitoring active patient records and data for accuracy, completeness and compliance.
Preparing and coordinating the distribution of patient information for on call staff.
Collaborating with finance staff to oversee accuracy of patient data and information, insuring that
medical information and certifications are timely and accurate.
Maintaining and ordering forms and record documents.
Assisting with the development and distribution of new and revised forms.
Providing work direction for Volunteers in Patient Records.
Reviewing and assigning ICD Codes.
Acts as Privacy Officer and oversees release of information duties.
Minimum Education: Associates Degree in Health Information or Medical Records
Technology preferred.
Minimum Experience: No prior experience is required, but some experience in maintaining a health
related organization's patient information and records is desired. Progressive experiences coordinating
patient records will also be desired.
