Medical Records Supervisor supervises routine medical record-keeping operations and healthcare information management to ensure secure, accurate, and reliable patient information management that complies with data and privacy regulations. Follows established policies and procedures to ensure effective and compliant record management, makes suggestions for process improvements. Being a Medical Records Supervisor implements digital technologies and tools to gain efficiencies, facilitate record retrieval, and ensure secure storage. Provides training for medical records staff and information resources to end-users. Additionally, Medical Records Supervisor coordinates with clinical and technical professionals to maintain robust records management systems and manage data for analysis and reporting. May require an associate degree. Typically requires Registered Health Information Technician (RHIT). Typically reports to a manager. The Medical Records Supervisor supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Medical Records Supervisor typically requires 3 years experience in the related area as an individual contributor.     Essential Functions of Social Services Supervisor / Medical Records:

    • Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels
    • Develop specific, measurable, realistic plans and objectives, which enhance social service daily
    • Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families
    • Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors
    • Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.  Include family in teaching as appropriate.
    • Demonstrate knowledge of clinical social work and discharge planning.  Interpret and promote residents’ rights and the “Residents’ Bill of Rights”
    • Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
    • Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
    • Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and center policy is followed.
    • Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
    • Be flexible, organized, and function under stressful situations.
    • Maintain a good rapport and working relationship with medical staff, departments, and employees.
    • Manage and operate equipment safely and correctly.
    • Treat patients and their families with respect and dignity.
    • Support and maintain a culture of safety and quality.
    • Maintains regulatory requirements, including all federal, state, local regulations, and accrediting organization standards
    • Always maintains resident confidentiality
    • Completes Inservice and returns in a timely manner
    • Attends annual review and department Inservice as scheduled
    • Actively participates in performance improvement and continuous quality improvement (CQI) activities
    • Maintain established medical record files; culls medical records in accordance with center and state-specific regulations
    • Pulls written medical record requests by established deadline
    • Maintains 100% accuracy of the medical record locator report to ensure it is complete and accurate and updates by end of week
    • Signs out medical records from the department using the medical record locator
    • Clears returning medical records from the medical record locator by end of shift
    • Files loose reports by the end of the shift
    • Maintains a high accuracy rate in filing of loose reports and resident records
    • Establishes and maintains supplies inventories
    • Files complete medical records in the storage area and department daily
    • Assists in retrieving medical records for physician completion
    • Is familiar with SNF and medical record documentation retention and schedules
    • Pulls verbal medical record requests within half hour of receipt
    • Answers telephone in a timely and polite manner
    • Participates in recording information for performance improvement activities 
    • Demonstrates knowledge of computer system
    • Willing to accept additional assignments
    • Supports and maintains a culture of safety and quality. 

    Qualifications of Social Services Supervisor / Medical Records:

    • High School Diploma or general equivalency is preferred
    • Bachelor’s degree in social work is preferred or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology
    • One year of supervised social work experience in a health care setting working directly with individuals is preferred
    • Current State Clinical Social Work Licensure
    • Accurate typing skills with a minimum of one year experience in medical records is preferred
    • Ability to work cooperatively as a member of a team
    • Ability to communicate effectively with residents and their family members, and at all levels of the organization
    • Ability to react decisively and quickly in emergency situations
    • Ability to maintain confidentiality
    • Ability to read, write and speak the English language
    • Knowledge of training techniques for clinical staff
    • Good Ethical Practice
    • Relationship management
    • Basic computer knowledge
