Medical Records Technician abstracts clinical information from medical records and assigns the appropriate ICD or CPT codes using industry-standard coding guidelines. Assigns required DRG (diagnosis-related grouping) codes. Being a Medical Records Technician works with coding databases and software to input and maintain data according to standard procedures. Performs quality audits of work. Additionally, Medical Records Technician maintains and up-to-date knowledge of coding and documentation requirements. Does not require a medical coding certification. Requires a high school diploma. Typically reports to a supervisor or manager. The Medical Records Technician works under the close direction of senior personnel in the functional area.   • SUMMARY:   Medical Records creates new medical records and retrieves existing medical records by gathering appropriate record folders and contents; assigning and recording new record numbers; verifying existing record numbers; inputting and recording locations to computer; delivering records.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • Gathers patient information by collecting demographic information from a variety of sources; interacting with registration areas and physicians' offices; retrieving information from automated printers.
    • Initiates the medical record by creating and processing the patient care record folder.
    • Maintains record availability by processing charts in the department; using chart mark-off procedures; facilitating chart location activities.
    • Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes.
    • Keeps health care providers informed by communicating the availability or unavailability of the record.
    • Communicates with the attending healthcare providers in a timely manner
    • Maintains quality results by following facility standards.
    • Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.
    • Maintains patient confidence by keeping patient records information confidential.
    • Serves and protects the hospital community by adhering to professional standards, hospital policies and procedures, federal, state, and local requirements, and JHACO standards.
    • Enhances medical records and facility reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
    • Maintains transportation schedules and records to ensure patients arrive at scheduled appointments

     

    QUALIFICATIONS:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

    •          High School Diploma or Equivalent required.
    •         Long Term Care Experience preferred.
    •          Ability to read, write, speak, and understand the English language.

    •          Required to sit, stand, bend, and walk regularly; lift and/or move up to 25 pounds.
    •          Visual and auditory ability sufficient for written and verbal communication.
    •          The noise level in the work environment is usually moderate.

     

    The Medical Records works under the direction of the Administrator to ensure all medical records are managed in accordance with Company policy and state and federal regulations. Responsible for the completion and accuracy of medical record information kept in both written and electronic forms and is responsible for ensuring that all protected health information is safeguarded as required by Company policy and HIPAA. The Medical Records works closely with the nursing department to ensure accurate and complete medical records are kept for all residents.

    Requirements:

    • Background in SNF preferred equivalent
    • Knowledge of HIPAA and release of information
    • Good computer skills
    • Able to recognize clinical document types and scan/import correctly into EMR
    • Must be able to sort and file alphabetically and numerically
    • Basic understanding of medical terminology
    • Displays positive customer relations skills
    • Must be reliable and flexible with schedule
