• Case Managers are responsible for the total management, skilled nursing care, and care coordination of assigned cases.
• Performs initial patient assessments and institutes the patient’s care plan at each visit and makes referrals as needed.
• Evaluate the patient’s needs, reviewing and updating the physician orders as required.
• Inform the physician, assigned nursing personnel and Clinical Supervisor of changes in the patient’s condition and needs.
• Prompt reporting of any and all incidents, patient/family/staff concerns, requests or needs to Clinical Supervisor.
• Provides instruction to other staff members, patient and family members or patient representative in accordance with the plan of care.
• Update PoC and Medication Profile every 60 days or as required.
• Ensure office chart and field chart are updated concurrently.
• Monitors documentation of discharge planning activities and assures that discharge summary is written in a timely fashion.
• Provides leadership and training to home health aides and other personnel.

Pay: $50.00 - $65.00 per year

Expected hours: 30 – 40 per week
