Hospice Social Worker – BSW / MSW
 
HealthCare Support is actively seeking a Hospice Social Worker – BSW / MSW to fill an opening with a Large Healthcare Network in West Palm Beach, FL.
 
Daily Responsibilities for Hospice Social Worker – BSW / MSW:

• The Social Worker will support patients and families going through Hospice care
• Daily responsibilities include assessing needs, collaborating with the medical team, designing proper plan of care, and ongoing communication with assigned cases to work toward goals
• 8-30am – 5pm
  • Sunday - Thursday OR Tuesday - Saturday

• Graduate of a Masters or Bachelors program – accredited by the Council on Social Work Education
• At least one year of experience in social after degree completion
• Experience in the medical setting as a social worker

• Hospice expereince

Pay Details: $22.94 - $28.68 per hours (based on years of experience) (in salary terms: $47,715-$59,654)
