To work in active partnership with all community providers and caregivers to ensure that all our patients receive the highest quality of care and support needed in helping them stay independent in the place they call home.
To assure that clients’ social service needs are adequately met, the medical social worker provides medical social services to clients in their place of residence under the supervision of the primary nurse coordinator and with orders of the primary physician
Assists the physician and other team members in understanding the significant social, emotional and financial factors related to the client’s health problems
Participates in the development of the plan of care. Develops and revises the patient specific plan of care in consultation with the physician, patient, patient’s representative (if any) and other care team members, based on individual needs of the patient and including patient specific measurable outcomes and goals
Participates in case conferences with other team members.
Prepares clinical and progress notes and submits all paperwork in a timely manner as per agency policy.
Works with the client’s family. Teaches the client/caregiver. Provides patient, caregiver, and family counseling and education.
Participates in discharge planning with the patient, caregiver, or patient representative (if any) and other team members.
Coordinates services in consultation with the home health staff. Communicates changes in the patient care plan to the physician.
Coordinates care delivery to meet the patient’s needs, and involves the patient, representative (if any) and caregiver (s) as appropriate, in the coordination of care activities.
Ensures that each patient, and his or her caregiver(s) where applicable, receive ongoing education and training provided by the Agency, as appropriate, regarding the care and services identified in the plan of care. Provides training, as necessary, to ensure a timely discharge.
Requirements:
Requires a Master’s or doctoral degree from a school of Social Work accredited by the Council on Social Work Education.
Requires one year of social work experience in a health care setting.
Good verbal, writing and interpersonal skills, and a thorough knowledge of the Medicare Regulations and Conditions of Participation for a certified Home Health Agency.
