Social Worker - LTC - Cranford, NJ

*Salary: Up to $65K/yr (depending on experience)*

Qualifications:

• Responsible for all admission and discharge aspects of facilities sub-acute and long-term population.
• Coordinate the admission of and assist in the initial adjustment for patients and their families/support unit.
• Provide on-going social work services to patients and significant others that conform to regulatory and professional standards.
• Coordinate the discharge planning with interdisciplinary team including therapy, nursing, homecare agencies and reimbursement coordinators.
• Provide continuous communication to families for the duration of the residents stay.

Qualifications/Requirements:

• Must have prior experience as a Social Worker
• Must have long term care experience
• Must have LCSW or CSW
• Must be a self-starter and energetic
• Proven Leadership & Management Skills
• Strong verbal & written communication skills
