    This is a Part-Time position working Saturday and Sunday from 10:00 - 18:30


    POSITION PURPOSE/SUMMARY:

    Provides psychosocial support and care coordination to patients, families, and caregivers and identifies and coordinates community resource outreach needs. Presents the benefits and appropriate use of Hospice of San Joaquin referral sources to include physicians or other healthcare professionals.


    CLINICAL SERVICES:

    Ensure that patient/family psychosocial issues are addressed and that community resource needs are identified for each patient/family.


    • Review and carry out care plan under the supervision of MSW consistent with Hospice of San Joaquin Policies and Procedures.
    • Contribute to the IDT in establishing the plan of care based on a complete and ongoing psychosocial assessment.
    • Provides direct clinical treatment services to patients/families, as indicated by diagnostic management and plan of care. Such services include but are not limited to case management and/or emotional support.
    • Provides clinical consultation to the IDT members, and health care staff and collaborates with community referral agencies/persons, observing confidentiality regulations and policies.
    • Identifies and coordinates community referrals required to support or enhance the treatment of patient/family.
    • Documents clinical services rendered to or on the behalf of the patient/family according to Hospice of San Joaquin’s policies and procedures.
    • May participate in monthly IDT meetings.
    • Maintains a professional appearance of self and work area; exhibits appropriate behavior with customers/participants and staff, i.e., respectful, courteous, supportive, and professional always.
    • Always maintains HIPAA compliance.
    • Additional duties as assigned by the Chief Clinical Officer or designee.
    • Develops early and appropriate referrals to Hospice of San Joaquin or the Palliative Care Program through the selection, acquisition, retention, and growth of business partnerships in the San Joaquin County.
    • Acts as brand champion, ensuring the mission and vision of Hospice of San Joaquin is understood and carried out with consistency.
    • Assists with the development and implementation of a strategic marketing plan to build sustainable relationships with referral customers.
    • Must excel at developing and maintaining relationships and creating memorable experiences for customers.
    • Creates and maintains relationships within the admissions department, other clinical personnel, and referral customers to assure effective internal and external communication.


    TEACHING AND SUPERVISION:
    Facilitates understanding of Hospice and Palliative Care for patients/families, staff, and volunteers.

    • Educates patients, families, and other professionals about Hospice and Palliative Care philosophy and services as defined by Hospice of San Joaquin’s Policies.
    • Participates in the training of patient care volunteers and appropriate community groups/agencies regarding the psychosocial and spiritual components of Hospice and Palliative Care.
    • Attends and participates in the in-service training provided by Hospice of San Joaquin quarterly.


    IDT COLLABORATION AND COMMUNICATION:
    Ensure continuity of care to patients/families by effectively collaborating and communicating with IDT members and the patient/family/caregivers.


    • Recognizes the knowledge, skills, and input of each discipline represented on the team and advocates for the most appropriate team member(s) intervention(s) with the patient/family/caregivers.
    • Communicates with IDT members and other agencies as needed to coordinate optimal care and optimal use of resources for the patient/family.
    • Identifies the need for consulting with other team members and volunteers regarding patient/family issues and follow through.
    • Functions as a patient/family advocate.
    • Interacts with IDT and other health care professionals to promote and enhance positive relationships.
    • Identifies and communicates specific Hospice and Palliative care learning needs to the Chief Clinical Officer and contributes to plans to meet these needs.


    LIASON/REFERRAL SUPPORT:

    • Provide Hospice and Palliative Care education – support of Hospice of San Joaquin and Palliative Care Program service information via telephone communication, physician office, hospital, health care agency, clinic, or home visits.
    • Generates timely referrals to Hospice of San Joaquin or Palliative Care Program through development and maintenance of successful customer relationships.
    • Conducts _____ (Determined by relationship type) sales calls per day to area physicians, healthcare professionals, agencies and/or facilities.
    • Creates account information (customer relationship management); documents service delivery- contact daily and maintains a consistent and efficient territory planner.
    • Communicates daily with Managers and Intake Supervisor to foster and maintain an environment of teamwork and required skills.
    • Utilizes Hospice of San Joaquin information systems for service contact documentation and provides statistical outcome data weekly/monthly.
    • Provides referral outcome information to referral sources in a timely fashion.
    • Documents customer satisfaction and utilizes feedback information for continuous quality improvement.
    • Maintains a professional appearance of self and work area; exhibits appropriate behavior with customers/participants and staff, i.e. respectful, courteous, supportive and professional at all times.
    • Adhere to all Hospice of San Joaquin Policies and Procedures to include Hospice of San Joaquin Compliance manual.
    • Must have a BA/BS degree in a human services field.
    • Knowledge of group process and ability to facilitate an IDT approach to clinical and administrative duties.
    • Skilled in both written and verbal communication.
    • Knowledge of community resources.
    • Knowledge of child and elder abuse reporting requirements and other significant legislation and laws relating to Palliative care practice.
    • Knowledge and understanding of the individual, child, and family development, human behavior and communication, the influence of culture and values in Palliative Care.
    • Computer literacy in word processing on the IBM compatibles preferred.


    PHYSICAL AND OTHER QUALIFICATIONS

    • Vehicle – Ability and willingness to drive own vehicle, have a valid California driver’s license, and have automobile insurance, which meets State standards. All employee volunteers, consultants, and consultant volunteers who drive need to provide Hospice of San Joaquin evidence of insurance. This is necessary to be covered by the agency’s non-owned auto policy.
    • Mobility – Frequent operation of a keyboard and sitting for long periods of time; occasional standing for long periods of time, walking, pushing/pulling, bending/squatting, driving, climbing ladders or stairs, and crawling.
    • Frequency of Lifting – Occasional lifting of 5 to 35 lbs.
    • Visual Requirements – Constant use of overall vision; the frequent need for color perception, hand/eye coordination, reading/close-up work, and field of vision.
    • Hearing/Talking – Frequent hearing of normal speech, hearing the telephone, talking in person and on the telephone; occasional hearing of faint sounds.
    • Emotional/Physiological Factors – Frequent public contact, decision making, concentration, and working alone; occasional exposure to emergency situations (including death), heights, and hazardous materials.
    • Ability to participate in the evaluation(s) of own performance by identifying strengths and weaknesses and by developing a plan to improve areas of weakness.
    • Environmental Conditions – Occasional exposure to noise, dirt, dust, smoke, fumes, poor ventilation, and cold and heat.
