POSITION SUMMARY
Works as part an interdisciplinary team to facilitate the delivery of services to patients/families. Coordinates the discharge planning process. Uses assessment skills to accurately identify patients’ needs and demonstrates the ability to provide safe, therapeutic environment for all ages, pediatric, youth, adolescent, adult, and geriatric patients. Participates in patient/family teaching. Uses teaching literature and techniques for different age groups and learning needs.
KNOWLEDGE, SKILLS & ABILITIES
Substantial sitting, walking and standing. Light lifting (not to exceed 25 pounds), bending and pushing objects. Frequent exposure to patient care area. At times may be subject to high levels of stress. Requires considerable reading, mental effort.
