As a member of the interdisciplinary team, provides social service support for pregnant patients/partners and adolescents/adults seeking reproductive and sexual healthcare. Works collaboratively with the Allen Women’s Health Maternal Health (Iowa’s Healthy Pregnancy Program/Title V) and Family Planning (Title X) teams to assist patients with addressing resources in the community, understanding the healthcare system, and identifying and addressing risk factors. This team member also provides support as needed to UnityPoint Clinics when appropriate.
