Responsibilities:
Team Select Home Care is looking for a Medical Social Worker to join our Team Select family. The Medical Social worker will assist physician, or intermediate care provider, and other team members in understanding the social and emotional factors related to the health problems. They will also help develop the plan of care and prepares clinical and progress notes. The Medical Social worker reports directly to the Assistant Director of Nursing.
- Help coordinate Agency's services with the services of other community resources; use community resources.

Participate in in-services and continuing education programs.

Be a consultant to other Agency personnel.

Be a resource person to employees, patients, physicians and other allied healthcare providers.

Develop a cooperative relationship and communicate with physicians.

Immediately report any accident, incident, lost articles or unusual occurrence to Director of Clinical Services/Nursing Supervisor.

Participate in staff meetings and patient care conferences as scheduled.

Maintain contact with community support groups and provide professional expertise.

Coordinate social services within the Agency.

Help develop the plan of care; ensure that the patient plan of care includes identified problems, interventions and goals.

Participate in community activities and promote community awareness and support.

Provide psychosocial services following established standards. Evaluate quality of psychosocial services provided.

Inform the RN of changes in the patient's condition and needs.

Schedule patient visits according to patient needs and as physician orders.

Observe confidentiality and safeguards all patient related information.

Attend staff meetings and patient care conferences as scheduled.

Participate in peer review and QM activities.

Report all incomplete work assignments to Director of Clinical Services/Nursing Supervisor.

Perform other tasks as assigned.
Qualifications:
- A person who has a master’s degree from a school of social work accredited by the Council on Social Work Education, and has one-year experience in a health care setting, or
-
Have a baccalaureate degree in social work, psychology, sociology or other field related to social work, and has had at least one-year experience in a health care setting (functions as an assistant), or
-
Licensed in the state in which practicing.

3 or more years’ experience, preferred.

One (1) year experience in home care, preferred.

Maintain current CPR certification.
