General Information:

Job title: Medical Social Worker

Schedule: Full-time, 80 hours per pay period; Monday - Friday

Weekend rotation: No Weekends

Holiday rotation: No Holidays (paid time off)

Position Summary:

The Medical Social Worker in Acute Care along with the Interdisciplinary Team assists and supports patients and their families to identity areas of need. Once patient needs are identified, education is provided about various resources and plans are arranged; all while serving as the patients' advocate and honoring their choices and rights.

Position Responsibilities:

• Assists with discharge planning in Acute Care.
• Discuss/arranges options, services, and resources with patient and family to help with areas of need.
• Attends interdisciplinary care rounds to coordinate care and identify needs for discharge planning.
• Completes appropriate documentation and paperwork for patients' charts.
• Minimal time spent assisting ED and clinic patients, Cancer Care, etc.
• Attend facility meetings and in-services.
• Attend outside committee meetings with other agencies such as DHS, ADRC, and other community staff/colleagues.
• Develops and maintains professional relationships as a social worker throughout the organization and in the community.
• Participates in community outreach events.
• Assist with Health Care Power of Attorney documents and other Advanced Directives as able.
• Perform other duties as requested.

Position Requirements:

• Bachelors or Masters of Social Work degree required.
• Hold and maintain Social Worker Licensure by attending continuing education courses.
• Adheres to professional Code of Ethics.
• 1 years of related work experience preferred.

Knowledge, Skills, & Abilities

• Intermediate to proficient use of computers.
• Basic knowledge of Medicare and Medicaid systems.
• Ability to maintain confidentiality.
• Self-starter with excellent compassion, interpersonal communication, and problem-solving skills.
• Excellent verbal and written communication skills.
• Accuracy and attention to detail.
