Medical Social Worker (MSW) - Home Care jobs in Moline, IL

We are seeking a dedicated and compassionate Licensed Independent Social Worker (LISW) to join our Care at Home team. As an LISW, you will play a vital role in providing comprehensive care to patients. This position offers a unique opportunity to make a meaningful impact on patients’ lives. Here are the key details:

Position Overview

Orientation:

• Participate in an 8-12 week base orientation with ongoing training throughout the initial 12 months.
• Travel to partner regions is offered, coordinated, and compensated to optimize the orientation experience.
• Utilize a Care at Home discipline-specific orientation roadmap for effective onboarding.
• Engage with primary and support preceptors to optimize the orientation experience.

Logistics of Position:

• 0.80 FTE Position:
• Monday to Friday; 8 AM – 5 PM (1-hour lunch break).
• Work schedule: 4 days per week, with the routine (standard) day off on Tuesdays (subject to change based on business coverage needs).
• no weekends or holidays

Exempt Position:

• Full-time exempt (salaried) position based on 32 hours per week (64 hours per pay period).

EHR (Electronic Health Record):

• Utilize Epic Ambulatory and Epic Inpatient systems.

Standard Home-Based Services Productivity:

• Follow an all-inclusive productivity model:
• 1 unit = 80 minutes; 6 units = 8 hours.
• Initial visits typically require 2 units.
• Subsequent visits typically require 1 unit.
• Therefore, expect 4-5 visits per standard workday.

Initial Service Area:

• Cover a radius of 25 miles from affiliate campuses.

Suite of Services (Not All Services Active in All Regions):

Care Management Home Visits (CMHV) and Post Discharge Home Visits (PDHV):

• Proactively support patients’ self-management capability.
• Provide preventative care to prevent health crises.
• Educate patients through Know-Do-Share documents.

Palliative Care at Home (PallCaH):

• Manage serious illnesses and support patients’ goals of care.
• Help patients avoid health crises.

Qualifications:

• Master’s degree in Social Work (MSW) or related field.
• Valid state licensure as an LISW.
• Compassionate and patient-centered approach.
• Excellent communication skills.
• Ability to work independently and collaboratively.
