Medical Social Worker (MSW) provides social work services to hospital patients and their families. Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems related to hospitalization. Being a Medical Social Worker (MSW) participates as a member of multi-disciplinary team and may assist patients in treatment planning. May contribute to the formulation of discharge plans. Additionally, Medical Social Worker (MSW) may take a leadership role on more complex cases. Requires a master's degree of social work. Typically reports to a manager. Overview
Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation.
Responsibilities
- Provides complete psychosocial assessments of patients and families.
- Provides counseling to individuals and families to help solve social and emotional problems.
- Coordinates community resources to address/meet identified needs.
- Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
- Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
- Organizes support groups.
- Documents objective, concise and pertinent clinical data in the medical record.
- Perform other duties as assigned.
- Master’s Degree in Social Work (MSW) required.
- Social work experience in an acute, home care, or ambulatory setting preferred.
- Strong organizational skills, verbal and written communications skills.
- Sound decision making and judgment.
- Time management skills.
- Negotiation skills.
- Proven ability to deal with conflict resolution.
- Comfortable with basic computer skills.
