Job Summary and Qualifications

We are currently seeking a Social Worker MSW to join our team. As our Social Worker, you will provide discharge planning and assistance with social services to the patient and their family. You will collaborate with the care team and other healthcare professionals to implement an interdisciplinary process for evaluating a patients' progress from admission through return to the community. • Masters of Social Work (MSW) required
• 1 years’ experience or MSW internship required
• Licensed Clinical Social Worker preferred
• Certification in Social Work preferred
• Critical thinking skills, service excellence and good interpersonal skills.
• Ability to read and comprehend written/verbal instruction
• PC Skills
