Medical Social Worker (MSW) provides social work services to hospital patients and their families. Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems related to hospitalization. Being a Medical Social Worker (MSW) participates as a member of multi-disciplinary team and may assist patients in treatment planning. May contribute to the formulation of discharge plans. Additionally, Medical Social Worker (MSW) may take a leadership role on more complex cases. Requires a master's degree of social work. Typically reports to a manager. This is a part time position working in the Case Management department at Northwest Medical Center.
Summary:
Provides assessment and crisis intervention in acute care settings; plans, organizes, and implements social work services for patients and families. Provides long term care planning for patients/families. Responsible for complex discharge planning. Intervenes as needed with hospital patients with mental health problems or substance abuse problems. Provides assessment with case management for pediatric patients.

Required: Masters of Social Work degree
Experience:
Required: Two (2) years acute clinical experience
Skills:
Able to handle crisis situations that require judgment critical to patient welfare; Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.
