Job Summary and Qualifications

Summary:

The Social Worker will provide direct assistance with the social and emotional needs of patients and families in the home care setting and provide education/intervention as indicated to create and implement a plan of care in conjunction with all disciplines.

Qualifications:

Required Education: Masters of Social Work

Required License: MSW; Valid Driver License required.

Preferred License: LCSW

Required Experience: At least five years’ experience providing clinical psychosocial assessments in medical setting

• Class C License
• Masters Degree
