Social Service Director

Are you passionate about making a difference in the lives of the elderly and those in need of long-term care? We are seeking a dedicated and compassionate Social Service Director to join our team at The Arthur Home, a leading long-term care facility.

This is a full time position.

Responsibilities:

• Direct, establish, and plan the overall policies and goals for the social services department.
• Conduct patient assessments, develop care plans, and assist patients and families with adjustment to their new surroundings.
• Oversee the delivery of high-quality services to individuals with behavioral health needs.
• Ensure regulatory compliance.
• Implement care plans, assist with discharge planning, and document patient progress.
• Maintain a community resource list and coordinate family meetings.

Requirements:

• Minimum of 3 years of long-term care experience.
• Strong leadership skills and the ability to resolve escalated issues.
• Excellent communication and interpersonal skills.
• Compassion, ethics, and able to work independently or as a team
