Medical Social Worker (MSW) - Nursing Home jobs in Elyria, OH

McGregor PACE (Program of All-inclusive Care for the Elderly), is a community based service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing Seniors to remain at home.

Qualified MSW candidates will manage a caseload of 55-62 senior program participants and families to attain the goal of remaining in the community with the support of our Day Health Center services. Must conduct initial assessments of PACE applicants, assist with Care Plans and Treatment plans. Communicates with community agencies, participants, and families on an on-going basis

Must possess strong assessment skills, ability to recognize mental and behavioral problems to create effective treatment plans.

Requirements:

Master's degree in Social Work required; LSW or eligible in Ohio preferred.

Minimum one year experience working with the frail elderly or older population required

Experience with dementia population, chronic illness, nursing home, adult day care or partial hospitalization, preferred

Previous experience in an interdisciplinary care setting, desirable

Reliable transportation with valid Ohio driver's license

Pre-hire criminal background check and physical required
