Medical Social Worker (MSW) - Nursing Home jobs in Norwich, CT

Vanderman Place, a privately owned skilled nursing facility in Willimantic CT, is seeking a Social Worker to join their team of highly skilled professionals. Responsibilities include but are not limited to:

• Develop preliminary/comprehensive assessments for each resident.
• Report any issues/ changes to the Director of Social Services.
• Encourage the resident/family to participate in the developmental process and review of resident’s plan of care.
• Develop a written plan of care for every resident that identifies all social problems and needs of the resident, followed by setting goals to be accomplished for each problem/need identified.
• Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner.
• Review nurse’s documentation to determine if the care plan in place is being followed properly.
• Assist with medicaid appplications.

License/Certification:

• Associates degree in social work or human services
• 1 year of nursing home social worker experience required
