Job Duties and Responsibilities:- Knowlwdge of completing the MDS assessments
- Schedule care conferences
- Documentation of social service assessments, care plans and progress notes in Point Click Care EMR system
- Experience in discharge planning for short term residents
- Refer residents and families to appropriate to appropriate social service agencies
- Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
- Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
- Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
What require the following for this position:- Experience in the long term care setting preferred
- Experience in Point Click Care EMS
- Must be a LSW or MSW
