Basin Home Health & Hospice seeks a part-time Medical Social Worker.

As a Medical Social Worker, you will support patients, families, and caregivers in their homes, helping them to cope with social and emotional issues and making practical arrangements relating to end-of-life. You will serve as an integral member of the healthcare team, providing assessments, coordination, treatment planning, information, and referral to community resources and other social work services to meet the complex needs of every patient and family.

Qualifications

• Master’s of Social Work (MSW) and licensure or Bachelor of Social Work and Licensure (Must be licensed in the State of New Mexico)
• Exceptional communication skills (multilingual is a plus )
• Exceptional professionalism
• Team-care oriented
• Experience working in geriatrics, hospice, or end-of-life care
• Valid driver’s license
