Meeting/Event Planner plans and organizes live or virtual meetings and events for an organization or external clients. Implements an event strategy to achieve objectives. Being a Meeting/Event Planner researches and recommends event venues. Coordinates meeting logistics, including attendee registration, transportation, accommodations, meals, and technology. Additionally, Meeting/Event Planner conducts rehearsals and system checks of all equipment and connectivity. Researches and maintains relationships with vendors for event support services. Coordinates budget planning and ensures that events stay within cost projections. Typically requires a bachelor's degree or equivalent. Typically reports to a manager or head of a unit/department. The Meeting/Event Planner occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. Responsibilities
The Discharge Planner is responsible for assisting and coordinating with the facilities Social Services Department to streamline the process for discharge planning.
Meeting the residents/guest on or shortly after the admission and following them until they discharge to the community.
Attend the 72 hour care plan meeting.
Ordering equiptment for the community setting.
Arranging home health nursing for after discharge, as needed.
Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family and the practitioner, assuring the resident/guest recieves the care that will meet their needs in the community.
Assist in procuring any items related to the discharge process for residents/guests anticipated to be returning to the community.
Qualifications
1-3 years of experience in a long-term care setting preferred
Social Work License
