 Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. The Certified Peer Specialist (CPS) is an active member of the team and provides peer support services to individuals with serious mental illnesses. CPS is responsible for the implementation of peer support services, which are Medicaid reimbursable under Georgia's Rehab Option. The Whole Health Action Management Program is a one-to-one service in which the Whole Health & Wellness Coach (CPS-WH) assists the individual with setting his/her personal expectations, introducing health objectives as an approach to accomplishing overall life goals, helping identify personal and meaningful motivation, and health/wellness self-management.

Looking for a Full-Time Certified Peer Specialist (CPS-MH, WHAM) to join and help build our Whole Health Action Management Program in Floyd County.

Job Description:

• Assist participants with setting personal expectations, introducing health objectives as an approach to accomplishing overall life goals.
• Help participants identify personal and meaningful motivation and health/wellness self-management.
• Support participants to be the directors of his/her health through identifying incremental and measurable steps/objectives.
• Facilitate health dialogues, explore multiple choices for health engagement
• Support individuals in overcoming fears and anxiety related to engaging with health care providers and procedures.
• Promote engagement with health practitioners including participating in annual physical.
• Assist individual in the work of finding a compatible primary physician.
• Participate in monthly meeting with WHAM nurses to staff cases and stay abreast of current health trends.
• Promote awareness regarding health indicators.
• Assist in understanding the idea of whole health and the role of health screenings.
• Support behavior changes for health improvement. Make available wellness tools to support individual's identified health goals.
• Teach/model/demonstrate skills such as nutrition, physical fitness, and healthy lifestyle choices.
• Promote and offer healthy environments and skills-development to assist in modifying own living environments for wellness.
• Facilitate multiple WHAM groups with up to 12 participants.
• Complete documentation per DBHDD standards.
• Complete all annual training assigned per HRH protocol.
• Demonstrate excellent verbal communication, written and listening skills.
• Attend all monthly meets as required.
• Complete CEUs as necessary to keep CPS/WHAM certifications current.
• Transport individuals as necessary.
• Other duties as assigned by program manager

Requirements

Job Requirements:

• High School Diploma or GED required.
• Must be a Certified Peer Specialist (CPS-MH).
• Must have Whole Health Action Management (WHAM) certification.
• Computer skills, organizational and teaching skills, ability to work with minimal supervision.
• Must be able to pass background investigation, drug screen and have good 7yr. motor vehicle report.
