Shift: Full-time; 40 hrs/week - flexible

Provide direct services at CMH Medical Clinic & Fredericksburg Medical Clinic for comprehensive evaluation, diagnosis, person-centered and/or family-centered treatment planning, and treatment of clients assigned.

• Provides continuity of care with other care providers for patient evaluations, recommendations, follow-up and treatment.
• Makes appropriate referrals based on screening protocols and assessed needs of clients.
• Provide counseling, support and follow-up to recommendations with identification and resolution to barriers contributing to access, adherence and insurance/financial issues.
• Contribute to on-going treatment plans, compliance with medications, treatment regimens; offering solutions with facilitation, follow-up and monitoring for psychosocial plans.
• Maintain effective working and collaborative relationships with psychosocial resources and available community resources and services to facilitate development and implementation of plans for patients.
• Develops appropriate new skills in Evidence Based Practice modalities and accepts new responsibilities as program and community needs evolve.

Minimum Qualifications:

• Master’s Degree in Social Work with licensure (LISW or LMHC)
• Licensed or able to complete licensing requirements to provide outpatient therapy services in the state of Iowa.
• Minimum of two years of experience.
• Must be able to read, write, type, speak and understand English.

Preferred training:

· Mental Health Counseling (MHC)

· Marriage and Family Therapy (MFT)

· EDMR (Eye Movement Desensitization and Reprocessing)

· PCIT (Parent-Child Interaction Therapy).
