Case Managers must have a bachelor’s degree in a human service related field including but not limited to psychology, education, rehabilitation counseling, or counseling from an accredited institution; and one year of paid experience in a human service field providing direct consumer services or case management or
A licensed registered nurse; and one year paid experience as a registered nurse in public health or a human services related field providing direct consumer services or case management; or
A bachelor’s or master’s degree in social work from a social work program accredited by the Council on Social Work Education;
Thirty hours of graduated level course credit in a human services related field may be substituted for the one year of required paid experience. Experience may be obtained before or after completion of the degree or obtaining licensure.
Responsibilities:
Communicate program information to parents effectively and clearly.
Thoroughly review referrals and perform pre-assessment activities.
Attend all required training sessions as required by agency and state office.
Make contact with families in the allotted time frame.
Secure and maintain appropriate release of information for each client.
Keep detailed records of every case, home visit, every referral and every contact with resources on the client’s behalf. Ensures that intake information is collected and elicits relevant additional information from parents and team members.
Network with other agencies to stay informed about community services and resources.
Respect confidentiality at all times, at home, in public, in committee meetings and in consultations.
Coordinate with medical and health providers. Follow up with other provider agencies to assure that assistance commitments are being met in a timely fashion.
Ensures that the team is adequately prepared for evaluation and assessment activities.
