Responsibilities Include:
Guides and directs and supervises the activities of the CNA's, CMA's and Nurses. Shares responsibilities for conducting admission assessments, MDS and care plans. Reviews, modify as needed, patients’ care plans in collaboration with patient, family/designated representative and the interdisciplinary team. Attends care planning and discharge planning review meetings. Provides patient/family education during stay and to prepare for discharge. Ensure that the rooms/beds assigned to new admissions, In-house transfer are ready to accept the patient. Conducts daily rounds. Communicates and report relevant and significant patient related information or changes in condition to the Physician, Nurse Practitioner , DON. Responds appropriately to patient care emergencies, provides guidance and direction to other caregivers in managing emergency events. Assumes responsibility and accountability for the provision of resident care according to professional standards of nursing practice in compliance with facility policies, state and federal laws and regulations.
