Responsibilities include reviewing and screening requests, calculating new premiums, issuing new policies and associated documentation. Being a New Policy Coordinator serves assigned accounts by responding to inquiries and resolving issues of concern. Requires an associate degree or its equivalent. Additionally, New Policy Coordinator typically reports to a supervisor or manager. The New Policy Coordinator possesses a moderate understanding of general aspects of the job. The Care Coordinator will be responsible for linking clients to medical services, assisting clients with navigation of medical services, assisting with addressing barriers to care for both behavioral health and medical care, monitoring client’s use of appropriate medical services, and following up with clients on issues relevant to receiving proper medical care. This position will be providing services in the clinic, in the community, and in client homes. Must have knowledge about the medical care system or have a relevant background to be able to attain such knowledge.
The Care Coordinator specifically working with transition-age youth (ages 14-26) will collaborate with other providers to meet the unique needs of these clients. They will provide community outreach to increase awareness of youth culture, development and work to reduce stigma related to behavioral health supports for transition-age youth.
The Care Coordinator specifically working with justice-involved consumers will collaborate with other providers to meet the unique needs of these clients. This Care Coordinator serves as an entry point for consumers referred to services by pretrial services division of the courts. This Care Coordinator will address the whole person care needs of the client and work to ensure a lowered risk of recidivism.
