Housing Manager

  • Case Management

    • Facilitates the development of clients’ individual action plans focused on income and housing goals
    • Meets twice weekly with clients to review action plan, to assess progress to date and to determine the need for any modifications to plan.
    • Facilitate monthly house meeting with clients to disseminate information, discuss house rules and other issues affecting the operation of the shelter.
    • Develops ongoing relationships with area service providers and works closely with providers to refer clients to and advocate on behalf of clients for supports and services that support the attainment and sustainability of clients’ goals including:

    o Transportation for appointments, interviews, employment
    o Substance abuse education & relapse prevention
    o Healthcare
    o Other services: legal aid, TANF, SNAP, HPRP, etc.
    o Education, schooling, school transportation, etc.
    o Childcare, early care & education, prenatal services, etc.
    o Recovery programs
    o Employment readiness (resume, interview soft skills), training, placement, and job retention
    o Housing programs and services.

    • Ensures a flow of on-going, two-way communication regular communication between case manager and service providers / program leaders
    • Ensures all agency required and legally required waivers and releases are signed for each client
    • Ensures exit interview are conducted, analyzes exit interview data and reports key findings to management.

    Data / File Management & Reporting

    • Develops the necessary data collection instruments for client files
    • Maintains up-to-date client files on daily basis, documenting client action plan and meetings on a daily basis.
    • Maintains and ensures the integrity of HMIS data in coordination with administrative assistant on daily basis to ensure accuracy, up-to-date data entry and on time reporting to funders.
    • Ensures the confidentiality of all client data – both hard copy and electronically submitted.
    • Provides calendar of appointments and workshops and client updates to and reviews with E.D on a weekly basis.

    Administrative Responsibilities

    • Performs other duties as assigned by the Executive Director

    QUALIFICATIONS

    Education: Bachelor degree and/or professional experience working with those who are homeless or at risk of homelessness or any equivalent combination of education and experience along with proficiency in Word, Excel, Access, PowerPoint and other software programs. Experience with HMIS preferred, but will train. Must have a valid and clean driver’s license.
    Competence: Strong organizational and communication (interpersonal, verbal and written) skills coupled with effective problem solving, decision making, multi-tasking skills, attention to detail and the ability to prioritize and facilitate the development of clear, concise short and long term plans of action. Proven ability to develop, maintain and utilize a strong network of service providers throughout SE CT to support clients’ goals.
    Experience: At least two years of experience in a related position and proficient in Word, Excel, Access, PowerPoint, and other software programs. Experience with HMIS preferred, but will train.

    Qualities: Team player with a positive attitude and a passion for transitioning those who are homeless to housing and self sufficiency. Person must demonstrate the ability to work collaboratively with agency staff and partners to achieve goals, to identify issues and seek resolution and to interact with clients in a compassionate, detached, calm, organized, respectful and professional manner. Person must be self assured and possess a working knowledge/interest in homelessness, workforce development/education, housing and housing programs, and mental health, domestic violence and substance abuse recovery.
