Nurse Manager - Phone Triage manages the day-to-day operation of a nurse triage call center and implements established policies, procedures, and service models. Uses performance and quality standards to achieve and maintain required care delivery levels. Being a Nurse Manager - Phone Triage monitors operations with call data analysis to identify trends, measure productivity, and respond to increases in demand for services. Conducts recruitment and selection activities to maintain optimum levels of nursing staff. Additionally, Nurse Manager - Phone Triage ensures compliance with nursing staff standards for competencies, assessments, licensure, certifications, and other annual training. Responds to escalated problems and may provide patient care evaluations and recommend responses. Typically requires a bachelor's degree in nursing. Requires Registered Nurse (RN). Typically reports to a director. The Nurse Manager - Phone Triage manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions.     Department Details

    Come join our Family Medicine team in West Fargo. We currently have 19 nursing team members - a mix of RN Care Managers, Triage RNs, LPNs, and PCTs. Typical hours are Monday through Friday 8:00am to 5:00pm. The role will work as a dual RN in Care Management and RN Triage work!

    Job Summary

    Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care. Collects and relays information (assessment, screening, diagnostic testing results and risk stratification) to providers on both hospital and clinic patients enrolled in the approved programs. Develops and provides individualized patient education based on assessed needs. Develops and implements individualized care plan. Assists with the coordination of patient care across the continuum by collaborating with the inter-professional team and community partners. Works with the clinical team and may manage the disease registries by identifying and following up with patients not meeting clinical and evidenced-based goals as well as Sanford approved clinical guidelines. Provides services that focus on health and wellness. Provides healthcare leadership, coordination of services, and the implementation of evidence-based practice to support the overall goals of the designated setting and/or program. Applies population health care management strategies to panels of patients.

    Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

    Qualifications

    Bachelor's degree in nursing required. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

    For Rural Health Network facilities only, employees who do not possess a Bachelor’s degree in the required field, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor’s degree in Nursing within five years of hire into position.

    For current employees in this role, leadership may have considered acceptable qualifications and work experience equivalency based on facility size, rural market, and business need, prior to new Sanford education requirements.

    Thorough knowledge of the nursing practice act and standards of nursing care, and a working knowledge of administrative and management techniques.

    Minimum of one year of professional nursing experience in a clinic care setting preferred. In-depth knowledge of specialty area preferred.

    Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.

    Basic Life Support (BLS) is required and must be obtained within six months of employment.

    Must possess a valid driver’s license.
