Nurse Manager - Phone Triage manages the day-to-day operation of a nurse triage call center and implements established policies, procedures, and service models. Uses performance and quality standards to achieve and maintain required care delivery levels. Being a Nurse Manager - Phone Triage monitors operations with call data analysis to identify trends, measure productivity, and respond to increases in demand for services. Conducts recruitment and selection activities to maintain optimum levels of nursing staff. Additionally, Nurse Manager - Phone Triage ensures compliance with nursing staff standards for competencies, assessments, licensure, certifications, and other annual training. Responds to escalated problems and may provide patient care evaluations and recommend responses. Typically requires a bachelor's degree in nursing. Requires Registered Nurse (RN). Typically reports to a director. The Nurse Manager - Phone Triage manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Nurse Manager - Phone Triage typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.     Qualifications and Skills:

    • Registered Nurse with a current license in the state of practice or compact licensure.
    • Two years hospice nursing experience
    • Excellent communication skills.
    • Excellent interpersonal skills.
    • Professional demeanor and appearance.
    • Current CPR certification.

    Responsibilities/essential functions:
    Must be able to perform the following essential job functions with or without reasonable accommodations.

    • Assess, plan, implement and evaluate the patient plan of care.
    • Performs initial and comprehensive assessments.
    • Develops a plan of care centered on issues, goals and interventions based on patients' goals for care.
    • Initiate's patient plan of care under the direction and orders of the physician.
    • Monitor and evaluate the plan of care in coordination with the interdisciplinary team.
    • Effectively manage patient symptoms in coordination with the physician.
    • Educate patients, families, and caregivers on techniques for in-home care and end of life care.
    • Presents patient clinical status and progression of decline in IDT meetings.
    • Timely notifies physician, nurse supervisor, and other interdisciplinary team members of patients' status, changes in condition and other relevant information.
    • Supervise the licensed practical nurse and effectively delegates assignments.
    • Trains and supervises hospice aide care.
    • Document at point of care, documentation is accurate and reflects patient condition and decline.
    • Completes education, participates in education in-services, continuing education programs and seminars.
    • Provides education to hospice employees and community partners.
    • Maintains compliance with Medicare Conditions of Participation, State and Federal laws, Arizona Nurse Practice Act, Joint Commission accreditation standards, and hospice policy and procedures.
    • Actively participates as a member of the team and demonstrates the organization mission and values in performance and behavior.
