Nurse Manager - Phone Triage manages the day-to-day operation of a nurse triage call center and implements established policies, procedures, and service models. Uses performance and quality standards to achieve and maintain required care delivery levels. Being a Nurse Manager - Phone Triage monitors operations with call data analysis to identify trends, measure productivity, and respond to increases in demand for services. Conducts recruitment and selection activities to maintain optimum levels of nursing staff. Additionally, Nurse Manager - Phone Triage ensures compliance with nursing staff standards for competencies, assessments, licensure, certifications, and other annual training. Responds to escalated problems and may provide patient care evaluations and recommend responses. Typically requires a bachelor's degree in nursing. Requires Registered Nurse (RN). Typically reports to a director. The Nurse Manager - Phone Triage manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. After Hours Triage Position Summary:
5 pm to 8 am; 7 days on 7 days off (Tuesday - Monday rotation)
Hybrid Remote Position: Combination of phone triage (90% est) and in person home visits (10% est)
What’s Required:
Active Registered Nurse license in the State of Michigan
Acute care, home care, or hospice experience is beneficial
Specific Duties:
Provides phone triage and after hours admissions/visits for home care and hospice patients during non-business hours.
Implements plan of care, coordinates services, and revises plan of care as necessary.
Performs initial and ongoing assessments of clients’ needs.
Serves as liaison between physician, hospital and agency personnel in assessing appropriateness of patient referral.
Counsels the client and family in meeting nursing and related needs.
