Nurse Manager - Phone Triage manages the day-to-day operation of a nurse triage call center and implements established policies, procedures, and service models. Uses performance and quality standards to achieve and maintain required care delivery levels. Being a Nurse Manager - Phone Triage monitors operations with call data analysis to identify trends, measure productivity, and respond to increases in demand for services. Conducts recruitment and selection activities to maintain optimum levels of nursing staff. Additionally, Nurse Manager - Phone Triage ensures compliance with nursing staff standards for competencies, assessments, licensure, certifications, and other annual training. Responds to escalated problems and may provide patient care evaluations and recommend responses. Typically requires a bachelor's degree in nursing. Requires Registered Nurse (RN). Typically reports to a director. The Nurse Manager - Phone Triage manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions.     0.7 FTE (28 hours): 10:30am to 7pm for 3.5 days a week (Hybrid Opportunity)

    JOB SUMMARY: The Triage RN is responsible for telephone triage and advice services for CommuniCare OLE patients and community partners (such as hospitals, urgent care centers, pharmacies, laboratories, diagnostic imaging centers, and other health care offices). The RN is responsible for answering the nurse triage line, which may include, but is not limited to, performing an assessment of patient symptoms, and referring them to the appropriate level of care (home care, appointment with primary care provider (PCP), and/or escalating treatment to higher level of care like the emergency department). The RN will collaborate with case management, nursing teams, providers, and other departments to ensure quality patient care.  The RN will conduct telephone triage following established procedures and protocols, and document calls as required.

    DUTIES AND RESPONSIBILITIES

    Monitors and responds to triage calls same day as scheduled by the call center

    Talks directly to patients on the telephone, by protocol performs an appropriate assessment, provides patient education and makes recommendations on next steps

    Provides appropriate home health advice to patients who do not need to go directly to the emergency department.

    Arranges appointments for patients who do not need to go to emergency department but need to see a PCP. Consults with PCP for clinical questions or other care team members as needed. Is responsible for placing patients in same day appointments or standby appointments for the day and the week.

    Recommend patients go to the emergency department for care, as appropriate

    Ensures accurate documentation of all consultations and treatments and records them in the patients’ record via the electronic health record (EHR) system in a timely manner.

    Prioritizes work tasks and services and completes tasks in a timely manner

    Assist Supervising Nurse with developing nursing policies and procedures and protocols, as needed

    Provides patient education regarding disease processes, therapies and healthful behaviors

    Reduce conflict and increase patient satisfaction through timely response to calls

    Perform other duties as assigned


     

    EDUCATION, EXPERIENCE, TRAINING

    1. Bachelor of Nursing degree from an accredited nursing school preferred.
    2. Minimum two (2) years of experience with direct patient care.
    3. Current State of California Registered Nurse License required.
    4. Must certify and remain current in CPR or BCLS certification.
    5. Experience in a community healthcare center preferred.
    6. Outpatient primary care setting experience preferred.
    7. Training in motivational interviewing preferred.
    8. Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
    9. Strong interpersonal and customer service skills required.
    10. Strong computer and data entry skills required; electronic health record system usage required.
    11. Strong analytical and problem-solving skills required.
