Transitions Chronic Care and Transitions Home Health Care were created in 2012 to address the fragmentation of care inherent to our healthcare system. Our Nurse Practitioners work both independently and in partnership with physicians, nursing staff, and other healthcare professionals to ensure optimal care across a variety of care settings. You will function as a chronic care/palliative care specialist with emphasis on managing and coordinating the complex chronic care and transitions of care for our patients as well as assisting the Home Health operations as needed. The Transitions Healthcare providers and support staff are passionate about the mission of this innovative and collaborative model of care. We understand the extra time and support it takes to care for patients with serious illness. You will have direct interaction with your patients delivered in a variety of locations that may include hospitals, skilled nursing facilities, assisted living facilities, long term care facilities and the patient’s home. You will help create personalized, long term care plans for our patients that maximize the dignity and the quality of life for our patients and their families.
Certified Nurse Practitioner (Preferred)
Primary Responsibilities:
Conduct in-home visits. Assessment includes:
Past medical history
Review of systems
Physical examination
Medication review etc.
Depression screening
Nurse Practitioner Responsibilities:
Performing physical exams and patient observations.
Recording patients' medical histories and symptoms.
Performing Transition Care Management visits
Creating patient care plans and contributing to existing ones.
Ordering, administering, and analyzing diagnostic tests.
Monitoring and operating medical equipment.
Diagnosing health issues.
Ordering medicine and other treatments.
Detecting changes in patients' health and modifying treatment plans, as needed.
Consulting with healthcare professionals and families.
Training patients and their families to manage and prevent illness and injury.
Checking vitals, conducting a physical exam
Educate members on topics such as disease process, medication, and compliance
Compliance with all HIPAA regulations and maintain security of protected health information (PHI)
Responsible for answering your patient’s calls.
Coordinating patient’s care with Home Health Agencies and staff
Collecting samples
Creating and implementing treatment plans
Performing small medical procedures
Administering treatments
Assisting Home Health operations as needed
Education and Experience:
NP license and national certification (ANCC or AANP) active, unencumbered licensure free from any open/unresolved disciplinary actions including probation or restrictions against privilege to practice. May be requested to obtain additional licensure supported by business
Family, Geriatrics, or Adult certification experience
Prescriptive authorization.
Experience providing primary care as a family nurse practitioner.
Proficiency with medical instruments and equipment required to perform your work.
Knowledge of computer-based data management programs and information systems, as well as medical records.
A sound understanding of all pertinent federal and state regulations, including HIPAA and OSHA.
Excellent verbal and written communication skills.
Knowledge of disease prevention.
Preferred Qualifications:
1 years of clinical experience in their highest level of education, clinical setting
Experience in gerontology, family, internal medicine, or endocrinology
Home Health care or home visit experience

Excellent administrative, organizational and verbal skills

Solid communication skills with the geriatric population
Proficient with electronic medical records, the internet and access to a personal computer
Ability to work independently

Ability to lift 30-pound bag in and out of car and up and down steps, as needed
Ability to obtain reliable transportation in order to complete home visit assessments
Ability to travel 1 hr away from your house (with notice, based on business needs)
Ability to navigate stairs and a variety of dwelling conditions and configurations
Ability to sit, stand and kneel, as needed, to perform physical assessment
