IHMG was founded in 2010 by Niccole Ranz, FNP-BC, with the goal to provide affordable, quality care to patients leveraging the medical expertise of Nurse Practitioners as primary care providers. IHMG offers eight service lines with the support of 70 Nurse Practitioners and their collaborating physicians.

In partnership with clients, the IHMG team manages the care of the most labor-intensive patients, provides medical oversight, education and training expertise, and ultimately improves patient outcomes by reducing preventable hospitalizations, readmission rates and administrative costs.

IHMG is seeking Nurse Practitioners for our SNFist service line that provides primary care services to patients in post-acute facilities.

Job Type: Full-time

Duties and Responsibilities:

• Complete all required documentation within 48 hours.
• Daily travel to multiple facilities
• Maintains current NP licensure as required by the State.
• Participates in learning activities to facilitate maintenance of current practice standards.
• Conducts required visits during regular facility business hours.
• Fosters collaborative working relationships with facilities visited.
• Reports any facility concerns or issues to Supervisor ASAP.
• Gathers documentation of needed pt information in HIPAA compliant manner as defined by IHMG.
• It is expected the NP will see 12-15 pts per day.
