Summary: Coryell Health is seeking a Family Nurse Practitioner/Acute Care Nurse Practitioner to provide primary and preventive care to patients of all ages in our critical access hospital and clinic. In this role, you will be responsible for conducting physical exams, diagnosing illnesses, developing treatment plans, prescribing medications, ordering tests, providing education, and coordinating care for patients.
Responsibilities:
- Perform comprehensive physical exams, order and interpret diagnostic tests to identify and assess patients' medical conditions
- Diagnose acute and chronic illnesses such as diabetes, high blood pressure, infections, and injuries through analysis of health history, symptoms, and test results
- Develop individualized treatment plans that include medications, therapies, referral to specialists, and patient education/counseling
- Prescribe medication therapy, treatments, and durable medical goods as appropriate
- Provide preventive care through health screenings, immunizations, and risk factor counseling
- Educate patients on disease processes, prevention strategies, compliance with treatment plans, and positive lifestyle choices
- Coordinate patient care with primary care physicians, medical specialists, and other healthcare professionals as needed
- Document accurate patient histories, exam findings, procedures, diagnoses, and treatment plans
- Comply with all regulatory, legal, credentialing, and compliance requirements
