    Description

    PURPOSE OF POSITION

    The primary purpose of your job position is to provide each of your assigned residents with routine daily personal care and service in accordance with current applicable federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure that the highest degree of care and/or services can be maintained at all times.

     

    DELEGATION OF AUTHORITY

    As Certified Nurse Aide, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

I understand that as a result of my employment, I may be exposed to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals and that the facility will provide me instructions on how to prevent and control such exposures. I further understand that I may be exposed to the Hepatitis B Virus and that the facility will make available to me, free of charge, the Hepatitis B Vaccination.

    I further understand that my employment is at-will, and thereby understand that my employment may be terminated at-will either by the facility or myself, and that such termination can be made without notice.

     

    ESSENTIAL JOB FUNCTIONS

    Direct Care

    • Knock before entering a residents room.
    • Identify residents before providing care, serving meals, etc., as necessary.
    • On a daily basis, review and follow ADL care plan directives as written prior to performing care to determine if changes have been made. Address questions with Team Leader and/or RN Supervisor.
    • Provide care efficiently and as required in the residents ADL care plan, including, but not limited to:
      • Bed mobility/transfers/repositioning
      • Bathing
      • Dressing
      • Grooming, including shaving male residents and keeping hair on female residents clean shaven (i.e., facial hair, under arms, legs, etc.) as instructed.
      • Nail care
      • Eating
      • Toileting and incontinence care (perineal care, catheter, skin integrity)
      • Hearing aids/glasses/dentures/mouth care
      • Transport residents to and from appointments/activities
    • Ensure that all nursing care is provided in privacy.
    • Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, ensure dentures are in place if applicable, raise bed, position tables, place garment protectors, take to/from dining room).
    • Serve food trays and set up trays.
    • Record the residents food/fluid intake. Report changes in the residents eating habits.
    • Keep residents water pitchers clean and filled with fresh water (on each shift) and within easy reach of the resident if applicable.
    • Serve between meal and bedtime snacks. Distribute hydration as required or directed.
    • Perform after meal care (i.e., remove trays, clean residents hands, face, clothing, take to bathroom, brush teeth, clean dentures, etc.).
    • Keep residents dry (i.e., change gown, clothing, linen, etc., when wet or soiled).
    • Change bed linens. Keep linens tight to avoid wrinkles forming under the resident.
    • Make beds (occupied and unoccupied).
    • Position residents per care plan. Turn bedfast residents at least every two (2) hours as care planned.
    • Check and report bowel movements and character of stools as instructed.
    • Collect urine specimens as instructed.
    • Provide daily indwelling catheter care.
    • Assist residents to walk with or without self-help devices as care planned.
    • Perform restorative procedures as care planned. Document as required.
    • Provide daily Range of Motion exercises.
    • Weigh residents as instructed and document on proper form.
    • Keep the nurses call system within easy reach of resident.
    • Answer resident calls promptly.
    • Conduct rounds as required. Ensure that residents who are unable to call for help are checked frequently.
    • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and/or as care planned.
    • Observe and report the presence of pressure areas and skin breakdowns according to procedure.
    • Report injuries of unknown source, including skin tears.
    • Assist residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.).
    • Assist with activities as requested or assigned.
    • Provide residents with reality orientation as instructed.
    • Watch for and report any change in room temperature, ventilation, lighting, etc.
    • Assist with the care of and provide comfort to the dying resident. Provide post-mortem care as instructed.
    • Treat all residents fairly and with kindness. Provide care and services in a manner which is respectful of residents rights and dignity and which allows residents the right to choose, including the right to refuse care/treatment.
    • Maintain the confidentiality of all resident and facility information and comply with applicable laws related to confidentiality (HIPAA).
    • Provide a safe, secure, and home-like environment. Exercise care and concern for the property of the facility and residents belongings.

     

    Communication

    • Participate in and receive the nursing report upon reporting for and before leaving duty.
    • Able to effectively communicate with and use appropriate interventions when providing care for residents with behavioral health issues including dementia. Review care plan interventions for mood, well-being, behavior, and activity needs.
    • Report all accidents, incidents, changes in the residents condition, and complaints/concerns and grievances made by the residents to the Team Leader/RN Supervisor.
    • Report all allegations of resident abuse, neglect and/or misappropriation of property.
    • Immediately notify the Team Leader/RN Supervisor of any resident leaving/missing from the facility.
    • Attend all staff meetings and/or read minutes from staff meetings.

    Documentation

    • Complete required documentation once care/treatment/restorative program is provided.
    • Record all documentation timely and accurately as assigned, legibly and accurately.
    • Maintain bowel and bladder, weight, intake and output records as instructed.

    Safety/Procedures

    • Follow established policies concerning exposure to blood/body fluids.
    • Check rooms for food articles (i.e., food in proper container and properly marked/dated, unauthorized food items, etc.). Report if food not in containers and properly marked/dated.
    • Follow established infection control measures in the performance of duties, including, but not limited to, hand hygiene, exposure to blood and body fluids, and standard precautions.
    • Follow transmission based precautions as instructed and required.
    • Follow established procedures in the use and disposal of personal protective equipment.
    • Keep floors dry. Report spills immediately.
    • Keep excess supplies and equipment off the floor. Store in designated areas.
    • Clean, disinfect and return all resident equipment to its designated storage area after each use.
    • Follow established smoking regulations. Report all violations.
    • Maintain compliance with facility policies and procedures and applicable regulations.
    • Maintain a professional demeanor and appearance by consistently complying with the facilitys code of conduct and appearance and hygiene policy.
    • Cultivate an atmosphere of mutual trust, respect, kindness and consideration. Promote collaborative relationships and teamwork, committed to the Carmelite Sisters and the facilitys Mission. Show respect and consideration toward each other, regardless of position.
    • Train or direct other employees and volunteers as requested.
    • Attend and participate in in-service education and on-the-job training as directed, especially mandatory in-services and department meetings. Be responsible to attain training hours required for position/discipline, especially required inservice training re: resident rights/abuse.
    • Respond to fire alarms as required (refer to Fire Safety Manual for employees responsibilities and those that are specific to your position.
    • Follow all safety procedures and precautions in the performance of your duties and when operating equipment. Report all problems with equipment to supervisor and maintenance as soon as possible. Report all safety violations.
    • Use only the equipment you have been trained to use that is necessary to do your job. Inform direct supervisor of any special needs.
    • Follow established regulations governing the use of labels and Safety Data Sheets (SDS) for hazardous chemicals within the department.
    • Use personal protective equipment as required by facility policy or by product instructions.
    • Follow established ergonomics policies and procedures governing lifting techniques, repetitive tasks, and the use of safety equipment and supplies to prevent work-related injuries and illnesses.
    • Report all unsafe/hazardous conditions to your supervisor immediately.
    • Use supplies in an efficient manner to avoid waste.
    • Participate in Quality Assurance/Performance Improvement (QAPI) studies, support and implement QAPI initiatives, and offer suggestions to improve resident care and the workplace.
    • Review Corporate Compliance educational materials, comply with Corporate Compliance directives, and report any Corporate Compliance issues according to established procedures.

    SKILLS & ABILITIES

    Education:

    Must possess, as a minimum, a high school diploma or equivalent.

    Must have completed a nurse aide training program approved by the Pennsylvania Department of Education in order to be eligible for testing or must be currently enrolled with the Pennsylvania Department of Health as an eligible nurse aide.

     

    Experience:

    On-the-job training is provided for those who meet the education requirements.

    Must have no substantiated findings of abuse, neglect or misappropriation of property on file with the Pennsylvania Nurse Aide Registry.

     

    Certificates & Licenses: Must be certified as a nurse aide by the Pennsylvania Department of Health. If not certified at time of hire, must complete this process within 120 days of employment.

    Other Physical/Sensory Requirements (with or without the aid of mechanical devices)

    • Must be able to move intermittently throughout the work day.
    • Must be able to read, write, speak, and comprehend the English language in an understandable manner.
    • Must be able to cope with the mental and emotional stress of the position.
    • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
    • Must meet the general health requirements set forth by policies of this facility which include a medical and physical examination.
    • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
    • Must be able to assist in the evacuation of residents during emergency situations.
