Piatt County Nursing Home, a 100-bed skilled nursing facility in Monticello, Il, is currently seeking a Restorative Nurse to join our team of ancillary nurses. The Restorative Nurse plans, develops and supervises the implementation of the Restorative program to assist residents with the goal of increasing or maintaining functional abilities at the highest possible level. Coordinating with the Therapy department, the Restorative Nurse provides ongoing teaching and in-services in rehabilitation.

The Restorative Nurse reports to the Director of Nursing.

As the Restorative Nurse you will:

• Evaluate new admissions and current residents
• Develop, implement, and document individualized restorative programs for all residents
• Assist, train and support residents and staff with individualized restorative programs
• Communicate with resident, family, and staff
• Participate in MDS resident assessment
• Attend care plan meeting with resident and family
• Communicate and oversee Restorative CNA's
• Other duties as assigned

Requirements:

Satisfactorily completed or willing to complete within 30 days of hire the restorative/rehabilitation course approved by IDPH

Licensed Nurse in good standing with the State of Illinois. Looking for an RN for this role.

Previous supervisory experience preferred
