Nurse Practitioner - Nursing Home examines and treats patients independently and in autonomous collaboration with other health care professionals in a nursing home setting. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Being a Nurse Practitioner - Nursing Home requires a master's degree. Typically reports to a manager or head of a unit/department. Additionally, Nurse Practitioner - Nursing Home requires Certified Registered Nurse Practitioner (CRNP) and state authority for advanced practice.   • Evolve Healthcare is seeking a Home Health Nurse Practitioner for an 11 week contract covering Kennebec County Maine. This is a rapid start position- Provider must be licensed in Maine. Must be able to complete onboarding in under 1 week. Must start online modules on 10/16 and ready to begin working in the field 10/26-no exceptions on start date.

    Guarantee of a minimum of 5 assessments per day at $120 per visit (please note this is an all inclusive rate; mileage etc.) $195 per assessment beyond the 5 per day. Position is M-F Days completing minimum 25 visits per week. Contract Duration 11 weeks.
    No Show: In the event a patient cancels the day of the assessment, provider is still paid for that assessment. Gross-$3,000 per week for 25 assessments.

    Description:

    • You’ll provide annual in-home assessments (IHA) for members of Medicare, Medicaid, and Affordable Care Act plans
    • Visits identify care gaps, health risks, and social determinants of health to help PCPs and health plans promote better outcomes
    • You will collect patient histories, document patient concerns, and provide health education. You are not responsible for lab tests, blood work, or treatment
    • Successfully complete the self-paced and live portions of the onboarding training in the specified time allotted.- $500 payout for modules
    • Ability to drive up to a 50-mile radius from home address to complete appointments.
    • Be able to complete 25 assessments each week
    • Ensure high quality assessments are performed to meet member satisfaction, health plan expectations and Emcara Health quality standards.
    • Document and submit chart notes within 24 hours of the visit taking place.
    • Address charting queries within 48 hours of receipt.
    • Maintain adequate communication with clinical leadership staff.
    • Attend periodic training sessions and meetings pertinent to the position.
    • Must be comfortable entering into personal living spaces for members scheduled.
    • Ability to lift 20 lbs.
    • Ability to bend or kneel for extended periods of time if needed while performing assessments.
    • Masters or Doctorate degree from an accredited NP program.
    • Home health experience (Preferred)
    • 1 year of in-home assessment experience (Preferred)
    • Current RN and NP licensure in the Maine
    • Board certified by the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP)
    • Current BLS certification - must be from an accredited entity
    • Valid unencumbered state drivers license
    • Valid proof of sufficient car insurance coverage for the state appointments are scheduled to be completed in.
