    Located in the Rogue Valley, White City allows easy access to Metro-area of Medford while remaining engrossed with nature. White City is an unincorporated community in Jackson County, Oregon, in the Rogue Valley. It is about nine miles north of Medford and 34 miles north of the California border.     The Specialty Care RN manages the activities of an identified medical subspecialty to provide quality, coordinated patient driven care while controlling costs and improving efficiency of clinical systems. In addition, the Specialty Care RN coordinates patient care and services across the continuum of care in collaboration with the interdisciplinary health care team.

    Duties may include, but not limited to:
    • Manage the administrative functions of medical sub-specialist providers, staff, fellows, residents and students needed for the provision of clinical care.
    • Manage procedural schedules and/or clinic matrices.
    • Identify systems problems and leading/participating on improvement teams to resolve problems or facilitate change/evaluate improvement outcomes.
    • Utilize care coordination strategies most appropriate for an identified group of patients.
    • Guide medical subspecialty to accomplish VA SORCC and Veterans Integrated Service Network (VISN) goals for continuity of care, cost efficiency, quality and utilization of resources.
    • Provide oversight for proper utilization of medicine admissions.
    • Review and aggregate data (i.e. utilization patterns, External Peer Review Process, variances, quality and performance, etc.), and reporting findings to team/division on progress toward goals; 8) providing staff education to reinforce current processes and/or implement new processes.
    • Monitor procedural efficiency including cancellation rates, utilization of procedural time and communication among interdisciplinary team members.
    • Provide patient/family education and/or oversight of patient education processes to assure that patients and families have the knowledge and skills they need for decisions related to care.
    • Participate in team rounds using care coordination strategies (if applicable), recommend alternative resources in providing care for the patient and to advocate for the patient and family.
    • Communicate and interact with the health care team in planning and providing care across all levels of the continuum as needed.
    • Serve as liaison for the identified medical subspecialty within VA SORCC, VISN 20, and community partners.
    • Provide pre-procedural assessments, as needed, to plan for quality care across the continuum.
    • Incorporate Whole Health in plan of care for the patient; and actively participating in their specialty's clinic when needed to improve patient care.

    Qualifications

    Basic Requirements:
    • English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
    • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
