Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance. Educates members and families regarding health maintenance and acute and chronic illnesses. Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Nurse Practitioner Job Requirements: Education: Masters degree in Nursing required. Licensure: Wisconsin Registered Nurse/RN license required. Family, Geriatric, or Adult NP certification is required. DEA registration required.                                         Advance Nurse Prescriber required. NPI and CPR & AED required                   WI Driver's License required Experience: minimum of 1 year recent clinical nursing experience with an at risk or elderly population.
