Client Care Coordinator

As a Client Care Coordinator your role is to manage all aspects of client services. If you have a passion for caring for others, friendly personality, professional attitude, strong work ethic and the ability to stay organized while handling multiple tasks? Primary Duties:

• Build trusting relationships over the phone by providing good listening skills and empathy.
• Meet with potential clients and family members to discuss their needs and provide solutions for care.
• Create care plans within WellSky system that effectively communicate client needs.
• Maintain ongoing communication and consistent follow-up with client, family, Care Professionals, and office team.
• Conduct introductions for new relationships with client and Care Professional.
• Perform quality assurance visits within a timely manner to support ongoing care needs.
• Critical thinking and problem-solving various issues with a professional demeanor to find the best resolution.

Secondary:

• Participate in on-call rotation with office team.
• Attending quarterly Care Professional meetings.
• Available for occasional community events.
• Perform any and all other functions deemed necessary.

Qualifications:

• Demonstrate a friendly, helpful attitude as a team player.

• Present a professional appearance and demeanor.
• Experience of operating office equipment.
• Proficient technology skills with Microsoft Office. Salesforce preferred, but not required.
• Provide excellent listening skills to build trusting relationships.
• Maintain confidentiality.
• Follow office standards, practices, policies and procedures.
• Ability to work independently to structure daily activities using time management skills.
• Excellent organizational skills with a high level of daily follow-up activities.
• Obtains a valid drivers license and auto insurance.
• Ability to pass criminal background and drug testing.
