Position Summary:
We are seeking a Case Care Manager to join our home care team. As a Case Care Manager, you will coordinate caregiving services to assigned Clients. You will develop individualized care plans for your clients with the end goal of ensuring a healthy & positive in-home lifestyle supported by our caregivers that empowers client independence. Leading recruitment efforts, you will interview caregivers for current and new cases, set and manage the schedules, follow-up with caregivers, and manage office administration tasks.
Position is 40-45 hours a week average - 8AM to 5PM Monday through Friday - Some On-Call after Hours required.

The Case Care Manager is a hybrid role, 2 to 3 days in-office at our Farmington Hills offices, 3 days remote. The Case Care Manager must be available and willing to be on-call in a rotation. This position may require occasional commutes in the Metro Detroit region on an on need basis for case assessments.
