JOB DESCRIPTION

• Demonstrates support for Stanleyview Home Care mission statement to promote quality, efficient, comprehensive, and effective client services.
• Participates in and supports Stanleyview Home Care’s performance improvement activities.
• Provides Personal Care Services Attendants services to clients as identified by Stanleyview Home Care in the plan of services.
• Prepares and maintains accurate and timely client notes, documenting interventions, client pain score, outcomes, and client response.
• Reports client condition changes.
• Maintains confidentiality of client, employee and Stanleyview Home Care operations.
• Meets productivity requirements established for Basic Personal Care Assistant.
• Practices safety, universal precautions, and infection control measures.
• Maintains a neat appearance and positive attitude with clients, family, and personnel.

ROLES & RESPONSIBILITIES

• Assist with bathing, dressing, walking
• Assist with Physical Therapy Exercises
• Ensure medications are taken appropriately
• Meal preparation for client and family on designated days
• Personal laundry for client
• Companionship for client
• Transport client to and from doctor and clinical appointments and taking notes as appropriate
• Maintain and manage doctor’s appointment whiteboard

QUALIFICATIONS

• Experienced care givers
• CNA/NA ( preferred )
• Valid Driver’s License
• Good Driving Record
• Clean Criminal Background
• Must provide 3 references
