Legacy Management Group is looking for a Clinical Central Intake Coordinator! This opportunity is located in our Baton Rouge office. To qualify for this position, you must have nursing home experience and be a licensed LPN or RN.
The goal of this position is to assess referrals for multiple skilled nursing facilities efficiently and systematically. The Clinical Intake Coordinator will assist in referral and intake procedures- creating and maintaining efficient practices. They will communicate with referral sources via email, computer system, phone, and fax. They will also communicate with the nursing facilities to ensure a smooth admission process. The ideal candidate must be organized, detail-oriented, and possess extremely effective communication skills. They must have a high degree of comfort with computer systems and applications such as Microsoft Teams, PointClickCare, etc. They must be able to manage and navigate multiple computer systems/ applications throughout the day. The ideal candidate is capable of prioritizing tasks of critical importance as well as multitasking. The Clinical Central Intake Coordinator must maintain a professional, positive outlook and be able to problem-solve when needed.
Qualifications:
* 3 years of experience in the Long-Term Care setting is REQUIRED
* Valid LPN or RN License in the state of Louisiana
Pay is dependent upon license type and years of relevant work experience.
