Nursing Supervisor - Nursing Home jobs in Champaign, IL

Piatt County Nursing Home, a 100-bed skilled nursing facility in Monticello, IL., is currently seeking a Dementia Care Unit Supervisor RN. The primary purpose of the Dementia Care Supervisor is to plan, organize, direct and supervise all therapeutic recreation and the nursing care implemented for the Dementia Care Unit within Piatt County Nursing Home.

The Dementia Care Unit Supervisor will report directly to the Director of Nursing.

Essential Job Functions:

• Plan and direct recreational/therapeutic programs for residents in the unit, present new programs to the Director of Nursing for approval.
• Supervise and train Dementia Care Unit Staff.
• Evaluate individualized needs of residents thru participation in resident assessments. Plan and implement programs and care plans to meet these needs.
• Attend care plan meeting
• See that all scheduled activities are maintained and that related resources such as materials are provided.
• Assists with meal time as needed.
• Ensures the report of changes in the resident’s behavior, condition, or problems to the family, Physician, and Director of Nursing.
• Maintains confidentiality, does not discuss information regarding he residents or facility out of the Nursing Home.
• Is knowledgeable of the policies and procedures of the Dementia Care Unit to include nursing

Education and Experience:

• RN
• One year supervisory experience
• Dementia Unit Experience
• Dementia Certification
